Anti-Xa directo oral. FA, TVP/TEP. Menor sangrado que warfarina. Sin requerir monitorización.

TEV / FA no valvularPO · cada 12 h

5 mg/12hmáx 10 mg/día

Reducir a 2.5 mg/12h si ≥2 de: ≥80 a, ≤60 kg, Cr ≥1.5. Suspender 24-48 h pre-cirugía (48 h si alto riesgo sangrado, ClCr <50).

Tratamiento TEV inicialPO · cada 12 h × 7 días

10 mg/12hmáx 20 mg/día

Carga 7 días, luego 5 mg/12h × 6 meses, luego 2.5 mg/12h profilaxis extendida.

INSUFICIENCIA RENAL

FA no valvularPO · cada 12 h

ClCr 15-29: 2.5 mg/12h. ClCr <15 o diálisis: NO recomendado.

Riesgo de sangrado mayor. Suspender 24-48h pre-cirugía. Hematoma espinal con anestesia neuroaxial.

Lactancia: 8.3 Females and Males of Reproductive Potential Females of reproductive potential requiring anticoagulation should discuss pregnancy planning with their physician. The risk of clinically significant uterine bleeding, potentially requiring gynecological surgical interventions, identified with oral anticoagulants including apixaban should be assessed in females of reproductive potential and those with abnormal uterine bleeding.
